Injury Report Form
Players injured during rec and travel baseball games or practices may receive some insurance coverage provided by the Township of Mahwah. In order to receive some form of coverage, reports must be filed with the Mahwah Rec Department within 90 days of the incident. Generally, insurance claims must be made with your primary insurance provider before filing a claim with the town insurance provider. If an accident occurs, parents/coaches should immediately complete the Injury Report Form and submit it to the Mahwah Recreation Department. Should a claim be indicated, the Insurance Claim Form is submitted directly to the insurance company, whose address is on the form. Instructions for completing the form are here.
